Despite the considerable advances of epileptology drug-resistant epilepsies consist about 30% among all forms of epilepsy. A case of successful using of a new antiepileptic drug lacosamide (vimpat) in the treatment of 31 years old patient with the resistant form of cryptogenic focal epilepsy with focal automotor and secondarily generalized seizures is presented. Authors represent the review of the literature devoted to efficacy and tolerability of lacosamide in the treatment of drug-resistant epilepsy.
REVIEWS AND LECTURES
Despite the considerable advances of epileptology drug-resistant epilepsies consist about 30% among all forms of epilepsy. Authors represent the review of the literature devoted to efficacy and tolerability of zonisamide in the treatment of drug-resistant epilepsy. The current review of studies devoted to efficacy and safety of a new antiepileptic drug zonisamide in the treatment of epilepsy is proposed. The mechanism of action and pharmacokinetic of zonisamide are described; the questions of efficacy and tolerability in the treatment of drug-resistant focal epilepsies and other types of seizures and forms of epilepsy are considered. The possibilities of the use of the drug in the treatment of comorbid disorders are considered
The current review of studies devoted to efficacy and safety of laser therapy in the treatment of neurological diseases in children is proposed. The author discusses pathophysiological background of this method of therapy, general therapeutic approaches and definite schemes of therapy in different diseases.
SUPERVISION IN MEDICAL PRACTICE
A case of chronic sphenoidal sinusitis in a girl of 9 years old is proposed; in clinical picture oculomotor dysfunction occurred (ptosis, strabismus divergent, diplopia, epiphora). The condition was masked by neurological symptoms, and so initial differential diagnosis was between 1) ocular form of myopathy (including mitochondrial diseases), 2) ocular form of myasthenia and 3) onset of multiple sclerosis. The definite diagnosis «pansinusitis» was proposed by neurologist only after attentive analysis of clinical symptoms and data of MRI, only since 1,5 year after beginning of the disease. This clinical case demonstrates the complexity of differential diagnosis of chronic sphenoidal sinusitis in children and necessity of developed clinical thinking for a doctor of every speciality
The article describes a clinical case of benign myoclonic epilepsy of infancy in a child with cerebral palsy. Prospective observation of a patient (2 years duration) with perinatal pathology and development afterwards of spastic-hyperki-netic form of cerebral palsy and occurring epilepsy in infancy, corresponding of electro-clinical characteristics of idio-pathic generalized epilepsy — benign myoclonic epilepsy of infancy. Probably, it is the first description of such clinical case in literature. The author noted the necessity of correct syndrome diagnosis of different forms of epilepsy in children with cerebral palsy
