Additionally, to assess the safety and tolerability of zonisamide treatment, the incidence and nature of adverse events (AE), and its impact on body weight and cognitive functions using the pediatric EpiTrack test after 3 and 6 months of therapy.</p> <p><bold>Materials and methods.</bold> A multicenter study was conducted involving 35 physicians from various regions of the Russian Federation. The study enrolled 115 patients aged 6 to 17 years inclusive (mean age: 9.95 years), including 52 % boys, with a confirmed diagnosis of focal epilepsy. Zonisamide (Zonegran, JSC “Firma EUROSERVICE”, Russia) was prescribed according to the manufacturer’s instructions as adjunctive therapy for focal epilepsy with or without secondary generalization. Epileptic syndromes for which zonisamide was initiated included: self-limited focal epilepsies of childhood, structural focal epilepsies, structural variant of Lennox–Gastaut syndrome, and genetic epilepsies. Individual patients could exhibit a combination of multiple focal seizure types. Zonisamide was administered as add-on therapy at a mean daily dose of 4.6 mg / kg. After 3 months, 16 patients dropped out of the study due to non-compliance with the study protocol. During these first 3 months, no patients discontinued the study due to zonisamide withdrawal. A total of 99 patients continued the study. After the subsequent 3 months (i.e., 6 months from the baseline), an additional 5 patients dropped out; thus, 94 patients completed the study. Among these 5 remaining dropouts, only 2 patients discontinued due to zonisamide withdrawal, while the others dropped out due to protocol violations. Consequently, the baseline cohort consisted of 115 patients, with a total of 21 patients dropping out (19 due to protocol violations and 2 due to zonisamide discontinuation).</p> <p><bold>Results. </bold>Efficacy of zonisamide treatment was observed in 83 out of 94 patients. The highest rate of complete seizure control was recorded in the early adjunctive therapy group, where zonisamide was prescribed as the second add-on medication. Five patients developed AE that required treatment discontinuation. In the remaining patients, AE occurred during the titration period and were deemed acceptable (decreased body weight and appetite in overweight patients). Concurrently, a reduction in migraine frequency was reported in one patient, a decrease in tic severity in one patient, and an improvement in hyperactive behavior.</p> <p><bold>Conclusion. </bold>Zonisamide demonstrates high efficacy and favorable tolerability. The incidence of AE is comparable to that of other modern antiepileptic drugs.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение. </bold>Проблема лечения эпилепсии у детей остается актуальной в связи с недостаточной эффективностью базовых препаратов, побочными эффектами и ограничениями клинических рекомендаций.</p> <p><bold>Цель исследования </bold>– оценить эффективность применения зонисамида, его влияние на частоту, тяжесть и типы приступов через 3 и 6 мес терапии, а также безопасность и переносимость терапии зонисамидом, частоту и характер нежелательных явлений (НЯ), влияние препарата на массу тела, когнитивные функции с помощью теста EpiTrack (детский) через 3 и 6 мес терапии.</p> <p><bold>Материалы и методы. </bold>Проведено мультицентровое исследование, в котором приняли участие 35 врачей из разных регионов Российской Федерации. В исследование вошли 115 пациентов в возрасте от 6 до 17 лет включительно (средний возраст – 9,95 года), из них 52 % мальчиков, с установленным диагнозом «фокальная эпилепсия». Зонисамид (Зонегран, АО «Фирма ЕВРОСЕРВИС», Россия) назначался согласно инструкции (в дополнительной терапии фокальной эпилепсии с вторичной генерализацией или без нее). Эпилептические синдромы, при которых назначался зонисамид: возрастзависимые фокальные эпилепсии детского возраста, структурные фокальные эпилепсии, структурный вариант синдрома Леннокса–Гасто, генетические эпилепсии. У одного пациента могло сочетаться несколько типов фокальных приступов. Зонисамид назначался в качестве дополнительной терапии в средней суточной дозе 4,6 мг / кг. Через 3 мес из исследования выбыло 16 пациентов из-за несоблюдения протокола исследования. В течение 3 мес ни один пациент не выбыл из-за отмены зонисамида. Продолжили исследование 99 пациентов. Спустя следующие 3 мес (т.е. через 6 мес от начала исследования) выбыло еще 5 пациентов, таким образом, завершили исследование 94 пациента. Из 5 выбывших только у 2 пациентов зонисамид был отменен, остальные пациенты выбыли из-за нарушений протокола исследования. Таким образом, первоначальное число пациентов – 115, выбыл из исследования 21 пациент (из них 19 пациентов выбыло из-за нарушений протокола, 2 пациента прекратили участие из-за отмены зонисамида).</p> <p><bold>Результаты. </bold>Эффективность при приеме зонисамида отмечена у 83 из 94 пациентов. Наилучший показатель полного контроля приступов отмечен в группе ранней дополнительной терапии, когда зонисамид назначался в качестве 2-го дополнительного препарата. У 5 пациентов развились НЯ, потребовавшие отмены препарата. У остальных пациентов НЯ наблюдались в период титрации дозы и были приемлемыми (снижение массы тела и аппетита у пациентов с избыточной массой тела). Также отмечены урежение приступов мигрени у 1 пациента, снижение выраженности тиков у 1 пациента и улучшение гиперактивного поведения.</p> <p><bold>Выводы. </bold>Зонисамид обладает высокой эффективностью и хорошей переносимостью.
