Spasticity in CP patients may lead to severe secondary bone changes, such as hip dislocation, severe foot deformities (equinovarus or planovalgus), scoliosis, severe multiple contractures of the upper limb joints. Despite being a good agent to relieve spasticity, botulinum toxin is not able to completely address the problems associated with deformities and movement disorders due to muscle rigidity and changes in muscle structure.</p><p>Reducing muscle rigidity using Longidase® (hyaluronidase azoximer) is an important stage of rehabilitation of CP children. Longidase® has a prolonged enzymatic activity and contributes to a pronounced antioxidant and anti-inflammatory effect. Longidase® also has antifibrotic properties since it mitigates inflammation and regulates the production of inflammatory mediators. Longidase® is capable of depolymerizing connective tissue matrix in fibrous granulomatous formations. Moreover, it suppresses the negative feedback reaction aimed at the production of connective tissue components, which is very important in case of muscle spasticity in both children and adult patients. Longidase® reduces tissue edema, thereby increasing the range of motion in joints, reducing contractures, and preventing their formation. The development of step-by-step rehabilitation, which includes botulinum toxin therapy at the first stage and Longidase® injections into spastic muscles at the second stage, will increase muscle elasticity and range of motion and facilitate orthosis in childhood, thus postponing surgery.</p><p>We report a case of CP in a 7-year-old girl who was treated in Saint Petersburg State Pediatric Medical University. The patient had regular courses of botulinum toxin A therapy to reduce muscle spasticity and prevent persistent contractures. In addition to that, the patient received a course of ultrasound-guided Longidase® injections into spastic lower limb muscles a week following botulinum toxin therapy. Muscle echogenicity was assessed using ultrasound examination. Spasticity in the upper and lower extremities was detected in more than one joint. Patient’s score on the Modified Ashworth Scale was 3 for the lower limb. Longidase® injections significantly increased the range of motion in the ankle joint and ensured smooth sliding of muscle groups during physical exercises. Longidase® therapy was considered because of the need for surgery, while parents wanted to try alternative treatment strategies to postpone surgery.</p><p>Thus, botulinum toxin therapy followed by intramuscular Longidase® injections reduced rigidity of spastic muscles and increased the amplitude of passive and active movements in the ankle joint, as demonstrated by goniometry and ultrasound, as well as changes in the perimysium and endomysium.</p></abstract><trans-abstract xml:lang="ru"><p>Актуальность реабилитации детей со спастической формой детского церебрального паралича (ДЦП) является наиболее значимой в связи с нарушениями движения, опорности и позы на фоне спастичности мышц различных групп. Спастичность при ДЦП может приводить к тяжелым вторичным костным изменениям: дислокации головки тазобедренного сустава, тяжелым деформациям стоп (эквиноварусной или плано-вальгусной), сколиозу, тяжелым множественным контрактурам суставов верхних конечностей. Ботулинический токсин, купируя спастичность, не способен полностью решить проблемы, связанные с деформациями и нарушением движений, в связи с ригидностью мышц и изменением мышечной структуры.</p><p>Снижение мышечной ригидности препаратом Лонгидаза® (бовгиалуронидаза азоксимер) важно как этап реабилитации детей с ДЦП. Обладая ферментативной активностью пролонгированного действия, Лонгидаза® способствует выраженному антиоксидантному и противовоспалительному эффекту. Обладая противофиброзными свойствами, препарат ослабляет течение фазы воспаления и регулирует синтез медиаторов воспаления. Благодаря указанным свойствам Лонгидаза® способен не только деполимеризовать матрикс соединительной ткани в фиброзно-гранулематозных образованиях, но и подавлять обратную регуляторную реакцию, направленную на синтез компонентов соединительной ткани, что очень важно при спастичности мышц как у детей, так и у взрослых пациентов. Именно Лонгидаза®, уменьшая отечность в тканях, увеличивает объем движения в суставах, уменьшает контрактуры или предупреждает их формирование.</p><p>Создание этапной реабилитации, включающей на первом этапе ботулинотерапию, а на втором – инъекционную терапию препаратом Лонгидаза® в спастичные мышцы у пациентов с ДЦП, позволит увеличить эластичность мышечной ткани и объем движений в суставах, что приведет к облегчению ортезирования пациентов в детском возрасте и даст возможность отсрочить хирургические вмешательства.</p><p>Авторы представляют клинический случай пациентки 7 лет, наблюдающейся в Санкт-Петербургском государственном педиатрическом медицинском университете и проходящей регулярные инъекционные курсы терапии препаратом ботулинического токсина типа А с целью снижения спастичности и ограничения развития стойких контрактур. Пациентке был проведен курс инъекций препарата Лонгидаза® в мышцы нижних конечностей через неделю после проведенной сессии ботулинотерапии в спастичные мышцы под контролем ультразвуковой навигации. Эхогенность мышц была оценена при помощи ультразвукового исследования. Спастичность в верхних и нижних конечностях наблюдалась более чем в одном суставе. Оценка по модифицированной шкале Эшворта (Modified Ashworth Scale) составила 3 балла в нижней конечности. После проведенной инъекции Лонгидазы® резко увеличились угол объема движения в голеностопном суставе и плавность скольжения мышечных групп при проведении лечебной физкультуры. Вопрос о применении Лонгидазы® встал в связи с перспективой оперативного лечения, предложенного хирургами, и желанием родителей использовать возможные альтернативные методы лечения для отсрочки оперативного вмешательства.</p><p>Таким образом, после проведенной ботулинотерапии с поэтапным введением препарата Лонгидаза® внутримышечно получен терапевтический эффект в виде резкого уменьшения ригидности спастичных мышц и увеличения амплитуды пассивных и активных движений в голеностопном суставе, что было подтверждено гониометрией и изменением структуры мышечной ткани (при визуализации на ультразвуковом аппарате), а также изменением перимизия и эндомизия.</p></trans-abstract><kwd-group xml:lang="en"><kwd>cerebral palsy</kwd><kwd>spasticity</kwd><kwd>muscle rigidity</kwd><kwd>botulinum toxin therapy</kwd><kwd>Longidase®</kwd><kwd>range of motion</kwd><kwd>rehabilitation</kwd><kwd>alternative to surgery</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>детский церебральный паралич</kwd><kwd>спастичность</kwd><kwd>ригидность мышц</kwd><kwd>ботулинотерапия</kwd><kwd>Лонгидаза®</kwd><kwd>объем движений</kwd><kwd>реабилитация</kwd><kwd>альтернатива операционной терапии</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Kurenkov A.L., Klochkova O.A., Zmanovskaya V.A. et al.
