The analysis of international publications suggests that sulthiame is considered as a first-line drug for the treatment of age-dependent epilepsy with central temporal spikes (rolandic epilepsy). Sulthiame is highly effective in children with epileptic encephalopathies manifesting with spike-and-wave activity during sleep, including Landau-Kleffner syndrome, as well as in patients with myoclonic seizures. The drug might be also effective in patients with other forms of focal epilepsy, including those resistant to therapy. The tolerability of sulthiame is higher that that of old antiepileptic drugs and even levetiracetam; thus, it is associated with a lower risk of treatment interruptions due to adverse events. Moreover, sulthiame can be used for behavioral disorders (such as hyperkinetic behavior, aggressiveness) and cognitive impairments. Sulthiame can be effective in patients with epilepsy and sleep apnea.</p></abstract><trans-abstract xml:lang="ru"><p>Авторы представляют обзор литературы, посвященной эффективности и переносимости сультиама в лечении различных форм эпилепсии. Обзор международных публикаций показал, что в мире сультиам рассматривается как препарат первого выбора для лечения возрастзависимой эпилепсии с центрально-темпоральными спайками (роландической эпилепсии). Сультиам высокоэффективен у пациентов детского возраста с эпилептическими энцефалопатиями, проявляющимися феноменом спайк-волновой активации во сне, включая синдром Ландау–Клеффнера, а также при миоклонических приступах, и может быть эффективен у пациентов с другими формами фокальной эпилепсии, в том числе резистентными к терапии. Лечение сультиамом сопряжено с низким риском отмены из-за плохой переносимости, и сультиам превосходит по этому показателю не только старые антиэпилептические препараты, но и такие новые препараты, как леветирацетам. Отдельной сферой терапевтического применения сультиама служат поведенческие (гиперкинетическое поведение, агрессивность) и когнитивные нарушения у пациентов с эпилепсией.
