Burdenko National Medical Research Center of Neurosurgery of the Ministry of Health of Russia from 2010 to 2016. 28 males, 36 females, age from 3 months to 18 years, on average 7,2 years. The pathology localization: temporal – 21 cases, occipital and parietal – 18, frontal – 25. Pathomorphology: 12 patients with focal cortical dysplasia (FCD) I, 20 patients with FCD II, 5 patients with FCD III, 4 patients with tuberous sclerosis, 20 patients with low grade tumors, and 3 cases else. The duration of diseaseis from 61 to 5081 days. Catamnesis (follow-up) ranged from 364 to 1877 days. There was four criteria for including patients in research: intraoperative EEG, presence of initial and control registration of EсoG, follow-up above 364 days. There are three hallmarks selected as significant concerning the efficiency of neurophysiological monitoring: 1) the positive changes in scalp EEG: decrease of interhemispheric asymmetry due to reduction of slow activity on the side of pathology; significant reduction of the unilateral epileptiform activity on the side of pathology and bilateral if exist; 2) the positive changes in EсoG: significant reduction of the regular epileptiform activity; 3) the absence or existence of residual epileptiform activity in adjacent to resected pathology sites. The statistical processing carried out with application of a Stat10 package.<bold>   Results. </bold>The more expressed positive dynamics of EEG were revealed in patients of smaller age already during operation. A correlation was found between the age of patients and the positive dynamics of intraoperative EEG. An inverse correlation was found between the duration of the disease and the positive dynamics of the scalp EEG during surgery (p = 0.1560). The EEG changes during surgery were shown more often with reduction of the seizure anamnesis. There was found out that residual epileptiform activity on ECoG occurred more often at patients with longer anamnesis of a disease and at patients with frontal localization of pathology. The residual epileptiform activity on ECoG was registered more often at patients with preservation of seizures (outcomes 2, 3, 4, 5 ILAE).<bold>   Conclusions. </bold>During resective interventions for symptomatic epilepsy in children the probability of positive changes was the more, the younger the patient was. Intraoperative ECoG is one of the useful tools in epilepsy surgery, but it does not definitely determine success. There was no statistically significant relationship between the presence of residual epileptiform signs on the ECoG in the cortex adjacent to the removal area and the outcome of surgical treatment for seizures. Patients in follow-up had three times fewer seizures in the absence of residual epileptiform activity on the ECoG of adjacent to the resected areas of the cortex. The dependence is statistically insignificant. Removal of FCD type III and tumors is accompanied by a more complete elimination of epileptiform activity than FCD I, the level of statistical significance of p = 0.0305.</p></abstract><trans-abstract xml:lang="ru"><p><bold>   Цель исследования</bold> – выявить взаимосвязь изменений интраоперационной электроэнцефалограммы (ЭЭг) и электрокортикограммы (ЭКог) с возрастом пациентов, длительностью анамнеза, локализацией и типом патологии, а также с исходами лечения.<bold>   Материалы и методы. </bold>В исследование включены 64 пациента с симптоматической фармакорезистентной эпилепсией, оперированные в ФгАУ «Национальный медицинский исследовательский центр нейрохирургии им. акад. Н. Н. Бурденко» Минздрава России в период с 2010 по 2016 гг. Из них 36 пациентов женского пола, 28 – мужского, возраст от 3 мес до 18 лет, в среднем – 7,2 года. В 21 случае выявлена височная, в 18 случаях – затылочная и теменная, в 25 случаях – лобная локализация эпилептогенного повреждения. патоморфологический диагноз подтвердил фокальную кортикальную дисплазию (ФКД) I типа у 12 пациентов, ФКД II типа – у 20, ФКД III типа – у 5, туберозный склероз – у 4, опухоли низкой степени злокачественности – у 20 пациентов, другие патологии отмечались в 3 случаях. Длительность заболевания составила от 61 до 5081 дня, продолжительность послеоперационного катамнеза по приступам – от 364 до 1877 дней. В анализ включены пациенты с наличием интраоперационной исходной ЭКог, контрольной регистрации ЭКог, с данными динамики ЭЭг в ходе операции. Как значимые с точки зрения эффективности нейрофизиологического мониторинга, были отобраны 3 критерия: 1) положительная динамика на скальповой ЭЭг: снижение межполушарной асимметрии, выражающейся в замедлении биопотенциалов на стороне патологии, увеличение регулярности ритмики; существенное уменьшение индекса эпилептиформной активности на стороне патологии и снижение нарушений билатерально-синхронного характера; 2) положительная динамика на ЭКог: значимое уменьшение индекса регулярной эпилептиформной активности; 3) отсутствие или наличие остаточной эпилептиформной активности, оцениваемое в прилежащих к зоне резекции участках коры. Статистическая обработка проводилась с применением пакета Statistica 10.<bold>   Результаты. </bold>У пациентов младшего возраста наблюдались более выраженные положительные изменения картины ЭЭг уже во время операции. Выявлена зависимость между возрастом пациентов и положительной динамикой на интраоперационной ЭЭг. Выявлена обратная корреляция длительности заболевания и положительной динамики на скальповой ЭЭг во время операции (p = 0,1560). Обнаружены различия в наличии остаточной эпилептиформной активности на контрольной ЭКог у пациентов в зависимости от длительности анамнеза эпилепсии. Хотя статистическая значимость этой тенденции не достигнута, но чем длительнее анамнез приступов, тем более выражена остаточная эпилептиформная активность. При лобной и  теменно-затылочной локализациях патологии чаще, чем при височной, наблюдалась редукция эпилептиформной активности на контрольной ЭКог. При лобной локализации патологии чаще регистрировалась остаточная эпилептиформная активность на контрольной ЭКог. Статистическая значимость не достигнута. Также у пациентов с сохранением приступов (исходы II, III, IV, V по шкале исходов ILAE) чаще регистрировалась остаточная эпилептиформная активность на ЭКог после резекции эпилептогенного повреждения.<bold>   Выводы.</bold> В ходе резективных вмешательств по поводу симптоматической эпилепсии у детей вероятность положительных изменений на интраоперационной ЭЭг тем больше, чем моложе пациент. Интраоперационная ЭКог является одним из полезных, но не определяющих успех инструментов в хирургии эпилепсии. Не обнаружено статистически значимой связи между наличием остаточной эпилептиформной активности в прилежащих к зоне резекции отделах коры на ЭКог и исходом хирургического лечения в отношении приступов. При отсутствии остаточной эпилептиформной активности в соседних с резецированными областях коры на ЭКог в 3 раза чаще наблюдалось прекращение приступов в катамнезе, чем их продолжение. Статистическая значимость не достигнута.
