A high proportion of comorbid disorders of the neurotic spectrum in patients with ITW was determined, so tranquilizing and anxiolytic therapy was indicated in half of the patients. Electromyography doesn’t revealed pathology of peripheral neuromotor apparatus. Follow-up monitoring for 2–6 years, showed complete spontaneous regression of violations of the pattern of the walk at 84.8 % of the patients of 7–8 years of life. The formation of contractures of the ankle joint in 5 patients required operative correction. The issues of therapeutic tactics (gypsuming, orthosis of feet, botulinum toxin therapy, physiotherapy) were discussed. The absence of evidence-based effect of therapeutic measures is confirmed.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение. </bold>Идиопатическая ходьба на пальцах стоп (ИХП) в детском возрасте является доброкачественным возрастзависимым расстройством нормальной формулы ходьбы.</p><p><bold>Цель работы </bold>– неврологический, катамнестический и электронейромиографический анализ пациентов с ИХП.</p><p><bold>Материалы и методы. </bold>Исследуемую группу составили 59 пациентов в возрасте от 1 года 4 мес до 15 лет (средний возраст 6,10 ± 3,54 года) с преобладанием лиц мужского пола (76 %) и дебютом нарушений в возрасте до 4 лет в 83 % случаев, нормальным анамнезом и отсутствием признаков органического неврологического дефицита.</p><p><bold>Результаты и выводы. </bold>При проведении дифференциальной диагностики были исключены детский церебральный паралич, спастическая параплегия, периферические невропатии, миопатии и другие заболевания как возможные причины ИХП. Выявлена высокая распространенность коморбидных расстройств невротического спектра у пациентов с ИХП, что определило целесообразность транквилизирующей и анксиолитической терапии в 50 % случаев. Электронейромиография не выявила патологии периферического нейромоторного аппарата. Катамнестическое наблюдение в течение 2–6 лет показало полный спонтанный регресс нарушений рисунка ходьбы в 84,8 % случаев к 7–8 годам жизни. Формирование контрактур голеностопных суставов у 5 пациентов требовало оперативной коррекции. Обсуждены вопросы лечебной тактики (гипсование, ортезирование стоп, ботулинотерапия, физиолечение).
